Job Title:Perm - Physical Therapist (Rehab)
Location: Peterborough, NH
Rate:$67,000/ year - $90,000/year+ Full Benefits
Shift Type: Days/No weekends or holidays; 37.5 hours/week (salaried position).



Job Summary:

The Physical Therapist provides evaluation and treatment services to pediatric through geriatric patients in accordance with departmental policies and professional standards of care. The role focuses on improving patients' functional abilities and quality of life through individualized care plans and interprofessional collaboration.

Key Responsibilities:

• Perform comprehensive evaluations to assess patients' physical therapy needs.

• Develop and implement customized treatment plans with clearly defined short- and long-term goals in collaboration with the referring provider, patient/resident, caregivers, and interdisciplinary team.

• Deliver effective therapy sessions and continuously evaluate treatment progress.

• Educate patients, families, and caregivers on treatment plans, exercises, and progress to support recovery goals.

• Ensure all treatments are age-appropriate and tailored to the patient's physical and cognitive capabilities.

• Maintain accurate documentation in accordance with departmental standards.

Qualifications:

• Bachelor's degree in Physical Therapy from an accredited program Required

• Current New Hampshire Physical Therapy license Required

• BLS (AHA) Certification Required

• 1 2 years of experience in a rehabilitation setting Preferred
